Introduction to Miami-Dade Permit Forms
Miami-Dade County administers construction permits through the Department of Regulatory and Economic Resources (RER) Building Division, with the Herbert S. Saffir Permitting and Inspection Center (PIC) located at 11805 SW 26 Street, Miami, FL 33175 (telephone 786-315-2000). The Permitting and Inspection Center operates a virtual service model with most customer transactions completed online through the RER Plan Status and Application Submission Portal at miamidade.gov. Standard forms are color-coded by application type, with the yellow Building Permit Application as the primary form, the blue Application for Plan Revision used for revisions and shop drawings, and the green Zoning Improvement Application for zoning-related improvements.
Yellow Building Permit Application
The yellow Building Permit Application is the primary permit application form used for new construction, remodeling, renovation, additions, garage conversions, layout and structural changes, repairs, and demolition. The form requires the property location, contractor information, project description, total square foot area, value of work to be performed, and selection of the applicable permit category. Common categories include Building Category 01 for commercial master permits, Category 02 for residential master permits, Category 15 for total demolition of detached structures, electrical categories for electrical work, mechanical categories for HVAC work, and plumbing categories for plumbing work. A completed Contact Sheet (also called Requested Reviews form) accompanies the building permit application identifying the disciplines requiring plan review.
Master Permit Application
A master permit application covers all phases of a construction project including the electrical, plumbing, mechanical, and roofing elements as integrated scope. Master permits are required for new construction and structures, remodeling, renovations, additions, garage conversions, layout and structural changes, repairs, and demolition of an existing building. Following master permit issuance, subsidiary electrical, mechanical, plumbing/gas, roofing, and window permits typically require separate application and payment. Master permit submittal includes the yellow Building Permit Application with the appropriate residential or commercial category, the completed Contact Sheet, and signed-and-sealed construction documents from a Florida-licensed architect or engineer with drawings submitted digitally signed and sealed per Miami-Dade Digital Signing and Sealing guidelines.
Subsidiary and Stand-Alone Permits
Subsidiary permits follow a master permit for individual trades including electrical, mechanical, plumbing/gas, roofing, and windows. Subsidiary permits may be obtained through the E-permitting for Contractors web application allowing qualified contractors to submit applications, pay fees, and print permit cards online without in-person visits to the Permitting and Inspection Center. Plan-review subsidiary permits use the Portal or in-person submittal with the yellow Building Permit Application indicating the trade category and the applicable Electrical Fee Sheet, Mechanical Fee Sheet, or Plumbing and Gas Fee Sheet. Stand-alone permits are single-trade permits issued without an underlying master permit, supporting limited-scope work in a single trade.
Blue Application for Plan Revision and Shop Drawings
The blue Application for Plan Revision is used for design changes or corrections to an already-issued permit, with revisions submitted any time before final inspection. Once a permit is finalized, it cannot be revised — a new permit is required for further changes. Revision submittal includes the blue form with a description of the revision, a completed Contact Sheet, and revised detail drawings signed and sealed by the design professional. Shop drawings for pre-manufactured materials including roof trusses and floor joists also use the blue Application for Shop Drawings with submittal through the Portal or in-person at the Permitting Inspection Center. Shop drawings carry their own applicable fees.
Green Zoning Improvement Application
The green Zoning Improvement Application (ZIP) is used for zoning-related improvements including specific signage categories such as balloon signs and painted wall signs (signs painted directly onto a wall). The Zoning Improvement Application is required in addition to the Building Permit Application for sign installation, with sign categories including Electrical 03 for illuminated exterior signs and awning signs, Electrical 27 for exterior or interior outline lighting and illuminated signs inside a building or mall, and Building 51 for exterior non-illuminated signs and copy changes. All signs over 24 square feet require calculations and a registered engineer's seal. Sign permits address compliance with Miami-Dade Code Section 33-86 (sign permits required) and Section 33-87 (sign construction standards).
Owner-Builder Permit Form
Owner-Builder permits under Florida Statutes Section 489.103 require an additional Owner-Builder form supplementing the standard Building Permit Application. Owner-Builder status allows property owners to act as their own general contractor for construction on their owner-occupied residence, with the requirement that the owner remain on the property for at least one year after the Certificate of Occupancy issuance and that work be performed by the owner personally without commercial construction activities. Owner-Builder status does not waive the engineering requirements for structural, mechanical, electrical, plumbing, and other disciplines requiring Florida-licensed professional preparation. The additional Owner-Builder form documents owner acknowledgment of the Owner-Builder responsibilities and limitations.
Minor Repairs Scope of Work List
Minor repairs not exceeding $5,000.00 in value of material and labor may be submitted with a Scope of Work List for Minor Repairs in lieu of full plans. Minor repairs are defined as restoring to good or sound condition a part or parts of an existing building for the purpose of its maintenance, and patching or restoration of damaged materials, elements, equipment, or fixtures for maintenance. Minor repairs explicitly exclude repair of structural damage, additions, alterations, and renovations. Applications may be denied if proposed work exceeds the minor repair criteria, and an inspector may determine during work that the scope exceeds minor repair criteria, requiring revision to the permit with full plan submittal. Properties under enforcement action require Enforcement Unit approval before minor repair filing.
Demolition and Asbestos Forms
Demolition permits require the yellow Building Permit Application with Category 15 for total demolition of detached structures (residential) or Category 02 for demolition attached to the main residence, and Category 15 (total commercial demolition) or Category 01 (attached commercial demolition). All commercial demolition and renovation projects over 160 square feet require asbestos review and approval from DERM (Division of Environmental Resources Management). Required submittals include a completed asbestos survey, the Affidavit for Asbestos Survey completed before plan review approval, and the Notice of Demolition or Asbestos Renovation form submitted to DERM at least 10 working days prior to start of demolition or asbestos renovation. Compliance follows Section 40 CFR Part 61 Subpart M NESHAP federal asbestos provisions.
Permit Extension and Reissue
Permit extensions provide an additional 180 days to obtain a valid inspection and may be requested before permit expiration or after expiration where the original permit still qualifies. Extension submittal uses the yellow Building Permit Application with the permit number indicated and 'Extension' selected under the Change to an Existing Permit section. Permit reissue applies to permits expired more than 90 days that no longer qualify for extension, providing 180 days to close the permit following reissue. Reissue submittal uses the yellow Building Permit Application with 'Reissue' selected and includes the original approved plans (obtainable through Building Plans and Microfilm at miamidade.gov). Process number extensions require email request to the Building Department with explanation that the application was being pursued in good faith.
